- Preferred email and phone number
- Program of interest (MSEM, MEM, Graduate Certificate program or Continuing Education course desired)
- Date you would like to enter the program, certificate or continuing education course
- Name of undergraduate degree and Institution; and, if possible
- A copy of your undergraduate transcripts where you obtained your undergraduate degree.
