OVID: Use Multi-Field search and select "Caption text" from the drop down to find images by words in captions. NOTE: OVID also includes a video/multimedia section at the top of  results lists.

PubMed: See images from articles (when available) in the right sidebar on results pages.

AccessMedicine: Look under the "Image" tab.
