Graduate Program Proofs

Please take the time to check your name and degree to ensure that the information that is being printed in the Commencement program is accurate. Be advised that your diploma/program name must match your name as it is listed in Banner. The only exception to this is if you want to use a middle initial rather than a middle name or vice versa.

Any other changes will require the completion of a Request for Name Change form, as well as legal documentation verifying your name.
